Title 40.1-51.1.(a), Code of Virginia: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: (a) On two Heidelberg-Harris 954 Web Offset Printing Presses located in the Press Room, the employer had employees exposed to caught-between hazards due to the employees cleaning hickies (blemishes) from the rollers or cylinders of the presses while they were still in motion vice when they were shut down as per the press operator's manual. "One feasible and acceptable abatement method, among others, to correct this hazard is to shut the presses down to clean hickies from rollers and cylinders as directed by the operators manual."s
