Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Press Brake Area- On October 10, 2001, the Chicago press brake #6 stroking control system was not set to operate in a single stroke mode, an employee sustained an injury while operating the press brake on continuous mode. The employer did not supervise the stroking selector mode. Among other methods, one feasible and acceptable method to correct this hazard is to include, but not be limited to the following: (1) Conform to the American National Standard Institute "ANSI" B11.3, Section 4.4.4.1. No abatement certification or documentation is required for this item.
