North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a)rebar work area -- a Wright 6000 pound hoist, used to lift bundles of rebar, was not provided with frequent inspections. Reference:ANSI B30.11-1998; Paragraph 11-2.1.2 Frequent Inspection. Frequent inspections shall be performed at intervals defined in paragraph 11- 2.1(b)(1) and shall include observations during operation. NOTE:Among other methods, one feasible and acceptable method to correct this hazard is to have the hoist inspected frequently by each operator before use.re

Reference:ANSI B30.16-1981; paragraph 16-2.3.3(b) Adjustment shall be made to assure correct functioning of components. NOTE:Among other methods of correcting this hazard is to repair the upper limit switch and ensure it is working properly before each use.
