Section 88.4 - Code of Iowa (2001) 88.4: Employer did not furnish to each of his employee's employment and a place of employment which is free from recognized hazards that are causing or are likely to cause death or serious physical harm. Employees were exposed to: (a)2217 W. 62nd Street North, Newton, Iowa - The employee amputated a finger and smashed another while operating a garbage truck compactor. The employer did not provide any training for employee(s) and does not have any manufacturers' operating instruction books on the 1985 Ford truck or the compactor manufactured by Leach Company, Oshkosh, WI, model #SCII 20 yd, serial no: SCII 1772. This exposed employees to hazards, such as amputations, lacerations, and death. Among other methods, one feasible and acceptable abatement method to correct this hazard is to train your employees on the truck they are operating. Referenced by ANSI Z 245.1-1984. This condition was noted on November 5, 2001.
