(a)At the Site - Employees are entering tunnels and crawl spaces to remove
and
installing piping.  The tunnels have not been identified as confined
spaces, the
hazards of the spaces have not been evaluated and safe entry procedures
have
not been established.  This could result in an employee being injured when
proper precautions are not followed.  Among other methods, one feasible and
acceptable abatement method to correct this hazard is to implement a
confined
space entry program such as, that described in American National Standard
Safety Requirements for Confined Spaces, ANSI Z117.1.
