Section 88.4 - Code of Iowa (1999) 88.4: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to unintended machine actuation: (a)Production Area - The shear is operated by a foot control pedal. The pedal is not protected from unintended actuation. This could result in an employee being injured should the machine operate unexpectedly. Among other methods, one feasible and acceptable abatement method to correct the hazard is to guard the pedal as required by the Safety Requirements for Shears, ANSI B11.4. (b)Production Area - The press brake is operated by a foot control pedal. The pedal is not protected from unintended actuation. This could result in an employee being injured should the machine operate unexpectedly. Among other methods, one feasible and acceptable abatement method to correct the hazard is to guard the pedal as required by the Safety Requirements for Press Brakes, ANSI B11.3.,
