Section 88.4 - Code of Iowa (2001) 88.4: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to shearing and pinching points without the benefit of Point-of-Operation Guarding: (a)Engineered Rubber Products Facility, The Plastic Molding Room, The 200 Ton Stationary Reaction Molding Machines - Employees are required to work in the point of operation when inserting the hot loaf of plastic into the die without the benefit of fixed perimeter guarding. One method of abatement is to install automatic injectors for feeding the plastic loaf into the die set. (b)Engineered Rubber Products Facility, The Plastic Molding Room, The 200 Ton Stationary Reaction Molding Machines - Employees are required to work in the point of operation when removing molded plastic pieces from the die set without the benefit of fixed perimeter guarding. One method of abatement is to install automatic ejectors for removing the molded piece from the die set. One method of abatement is to install any combination of perimeter guarding, presence sensing devices and/or two-hand controls to prevent entrapment in shearing and pinching points. These violations were observed on or about April 02, 2002.

Section 88.4 - Code of Iowa (2001) 88.4: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to shearing and pinching points without the benefit of a "Safe Distance" safeguarding program: (a)Engineered Rubber Products Facility, The Plastic Molding Room, The 200 Ton Stationary Reaction Molding Machines - Employees are required to work in the point of operation when inserting a loaf of molten plastic onto the lower platten of the die set without the benefit of a "Safe Distance" training program. (b)Engineered Rubber Products Facility, The Plastic Molding Room, The 200 Ton Stationary Reaction Molding Machines - Employees are required to work in the point of operation when removing molded plastic pieces from the die set without the benefit of a "Safe Distance" training program. Among other methods, one feasible and acceptable abatement method is to develop a "Safe Distance" exposure prevention procedure and training. Reference OSHA Directives; CPL 2 - 1.25 "Guidelines for Point of Operation Guarding". This violation was determined to exist on or about April 02, 2002.
