Section 88.4 - Code of Iowa (2002) 88.4: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential projectiles created by unsecured compressed gass cylinders being knocked over. NFPA 51B 4-5.7 (Fire Prevention in Use of Cutting and Welding Processes) incorporated by reference, requires compressed gas cylinders to be secured so they can not be knocked over. Among feasible methods to secure welding cylinders; position at a wall and place a chain attached to the wall around the cylinders: (a)Warehouse Tire Shredding Area, Southwest Exit - Approximately five welding gas cylinders were free standing near shredder #3, the final shredder of three. These cylinders were within fifteen feet of the southwest exit door. Several employees work in this area, monitoring the shredding process and cleaning up tire fluff.
