88.4:  The employer did not furnish employment and a place of employment
which was free
from recognized hazards that were causing or likely to cause death or
serious physical harm
to employees in that employees were exposed to:
(a)At the Facility - The employer did not comply with the above, in that
the
employer did not train employees in the safe use of ladders needed for use
at
the work place. Employees used a 6 foot step ladder to access a material
storage rack that the top shelf was 10 foot 10 inches to the bottom of the
shelf
and materials were stacked another 18+ inches higher. This condition
created
a hazard for materials falling on employees, and a hazard of employees
standing above the safe standing distance of the 6 foot step ladder and
falling
from the ladder. This condition was noted on or about 8/13/02.
Among other methods, one feasible and acceptable abatement method to
correct this hazard could be to train all employees in the proper
selection, use,
placement of ladders needed for work, and the nature of fall hazards in the
work place.s
