Two employees were not protected from fall hazards of
approximately 15' to ground elevation from the basket of a
Hercu-Lift UNO 41 E aerial lift (serial number 9731930697).
The employees were performing electrical maintenance on the
Itasca State Bank Sign at 1215 Pokegama Avenue South in
Grand Rapids, MN, without the use of personal fall arrest
systems.
