Stat. 182.653 subd. 2: The employer did not furnish to each employee, conditions of employment and a place of employment free from recognized hazards which caused or were likely to cause death or serious injury to employees; specifically: Appropriate fall protection was not provided for the elevated forklift work platform, which was observed in use on this jobsite. Guardrails were not provided for an exposed side of the platform. An employee employee exposed to fall hazards was not using a safety harness and lanyard while working from the platform.
