causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to:
On or about January 2, 2002, at the Alltel telecommunication tower site
located
off Highway 284 on County Road 409 in Forrest City, Arkansas;
(a)Three employees were allowed to be hoisted up the tower while all were
simultaneously attached to a single manila rope line.  When the employees
reached the approximately 100 ft. level, the rope line broke and exposed
the
employees to the hazard of falls from elevations.
(b)A single manila rope run through pulley blocks and attached to the
front bumper
of a pickup truck were used to hoist employees to their work areas on the
tower,
exposing employees to the hazard of falls from elevations.
(c)The manufacturer's specifications applicable to the hoisting system
that was being
used to elevate employees to their work areas on the tower was not
available and
the hoisting system was not based on the determination of a Registered
Professional Engineer.nal
