Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falling: In the shot blasting booth; the floor grate was not level throughout the booth; a foot of the 10' fiberglass stepladder was subject to be placed in a depression causing the ladder to be unstable and subjecting employees to falling hazards. Among other methods, one feasible and acceptable method to correct this hazard is to replace defective floor grating in the shot blast booth.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were subject to be crushed or strike their head on the floor if a forklift rolled over: a. On the west side of bay 21; the operator restraint system, which included a seat belt, had not been maintained on the Clark forklift J9. One feasible and acceptable method to correct this hazard is to reinstall the operator restraint system. b. In the east end of bay 9; Caterpillar forklift J1 did not have an operator restraint system. One feasible and acceptable method to correct this hazard is to install an appropriate operator restraint system.nt
