At the jobsite, maintenance shop; employer used a roll off truck with a
hydraulic Hoist GH
system to raise the front or rear of trucks high enough to remove the
transmission and rear ends.
Employees stand beneath this hydraulic hoist system being used for an
operation other than its
intended purpose of loading and unloading dumpsters, exposing employees to
the hazard of being
struck by falling truck.  Among other methods, one feasible and acceptable
method to correct
this hazard is to use a truck lifting device such as, but not limited to,
hydraulic lift in accordance
with industry practice on lifting and sustaining the load during
maintenance.
Per Informal Settlement Agreement dated 3/10/2003, verbiage changed as
follows:
At the jobsite, maintenance shop; employer used a roll off truck with a
hydraulic hoist GH system
for such operations of raising the front or rear of trucks to remove the
transmission and/or rear
ends, which was not the intended use of the roll off truck.  Among other
methods, one feasible
and acceptable method to correct this hazard is to use a truck lifting
device or other hoisting
apparatus.

disassembly and reassembly for cam actuated brakes and proper tools for
brake shoe installation
in accordance with industry practice.

training for new
employee(s) involved in brake repair.  Among other methods, one feasible
and acceptable method
to correct this hazard is to assure all training provided to employee(s)
