General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm: Employee(s) was exposed to the hazards of gas well blowouts, fire and/or derrick collapse; employee(s) working as the stabber on a casing crew was positioned on the derrick of an oil/gas drilling rig with only one means of egress, the derrick ladder. The use of the derrick ladder could be blocked by the preceding hazard. A secondary, emergency only, means of egress to remove the employee(s) from the proximity of the hazards could limit exposure. Among other methods to provide a secondary means of egress would be the installation and expeditious use of a properly rigged and secured "Geronimo" line. Employee(s) performing the stabbing function was exposed to the noted hazards while working on Helmerick and Payne Rig #158 at various locations in Latimer County, Oklahoma.
