Instance a)  On or about 2/19/02, the department supervisor was operating
a Hyster sit-
down fork lift in bay one was not wearing a seat belt.
Instance b)  On or about 2/19/02, the double column hem saw/ computer
controlled miter
saw operator was not wearing a seat belt while operating a Hyster 80 XL2
sit-down fork
lift, serial number G0050008319T.
Instance c)  On or about 2/21/02, an employee operating a sit-down fork
lift in bay 2 was
not wearing a seat belt.
Instance d)  On or about 2/20/02, an employee operating the "Big Red"
forklift, serial
number S-Z5-22182, in the yard behind the facility was not wearing a seat
belt.
Among other means an acceptable and feasible means of abatement is as
follows:
Provide and require the use of seat belts during operation of powered
industrial trucks.
The employer is not limited to the recommended abatement method.  It is
possible some of the
methods may not be effective in all circumstances.
