General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards: (a) Trinidad Benham Corp. at 139 E. Chestnut, Sterling, CO 80751: On or before Feb. 8, 2002, the employer did not install a door interlock on the personnel elevator located in the mill area, thereby exposing employees to a six foot or great fall hazard. The requirement of an interlock is established in ANSI Standard 2.12.2.1. Among other methods, one feasible and acceptable abatement method is to install electrically-driven doors that are interlocked to the position of the elevator car. These interlocks would limit access to the shaft to circumstances when the elevator car is at the level of the person desiring to enter the shaft. Abatement Note: Abatement certification and documentation are required for this item (See enclosed "Sample Abatement-Certification Letter").
