employees were exposed to fall hazards from an elevated personnel platform:
a)Strong Framing, Inc. at 191 S. McCaslin in Louisville, CO: On or before
08/06/02, the employer did not ensure that a rough terrain, Skytrak 9038
Turbo, fork-lift with an employee occupied working platform was attended by
the operator.  This condition exposed employees to a fall from elevation of
approximately 13.5 feet.
Abatement methods should be in accordance with ASME/ANSI B56.6-1992,
Safety Standard for Rough Terrain Forklift Trucks.
