recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to hazards associated with unsafe use of or unsafe
condition of an
articulating boom platform:
(a)Hayden Construction at 1000 Broadway, Denver, CO 80404:  On August
10, 2002 the employer did not ensure the  Genie S-40 G3693HZ was used
safely in that the operator did not conduct the Pre-operation Inspection,
Function Tests, and Work Place Inspection. The failure to conduct these
inspections and tests exposed the employees to an unsafe articulating boom
platform and/or unsafe work conditions.
Among other methods, one feasible and acceptable abatement method to
correct this hazard
is to:
(a)Conduct Pre-operation Inspections as detailed in the Operator's Manual
for the
Genie S-40.
(b)Conduct Function Test as detailed in the Operator's Manual for the
Genie S-
40.
(c)Conduct Work Place Inspections as detailed in the Operator's Manual for
the
Genie S-40.
