Section 5(a)(1) of the Occupational Safety and Health Act lf 1970 did not furnish employment and a place of employment which were free from recognized hazards that were causing of likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards and/or the hazard of using unsafe below-the-hook lifting devices, resulting in falling loads: a)Process Fabricators, Inc. & 15704 Hwy 14, Sterling, CO: On or before November 13, 2002, the company did not ensure that the below- the-hook lifting devices used by employees to lift sheet steel stock were adequate. The 24' and two 12' spreader beams (supporting lifters) used in support of various overhead hoist lifting operations in the fabrication bays were not adequately; identified/marked, inspected, and load tested as required in ASME B30.20a-2001 The American Society of Mechanical Engineers - An American Standard - Below-The-Hook Lifting Devices: 1.The spreader beams were not marked with the manufacturer, serial#, lifter weight, and rated load as required by ASME B30.20a-2001, paragraphs 20-1.2.1(a) and (b). 2.Periodic visual quarterly inspections of the spreader beams (inspections of apparent external conditions to provide a basis for a continuing evaluation) were not recorded as required by ASME B30.20a-2001, paragraphs 20-1.3.1 and 20-1.3.3. 3.The spreader beams were not tested by a qualified or designated person prior to initial us to verify compliance with applicable provisions of ASME B30.20a-2001 as required in B30.20a-2001 paragraph 20-1.3.8 to include the initial load test requirements in paragraph 20-1.3.8.2. Abatement Note: One feasable abatement method is to mark, document inspections, and test the three spreader beams used in the fabrication bays in accordance with ASME B30.20a- 2001. Abatement note: Abatement certification is required from this item (see enclosed "Sample Abatement Certification Letter").
