Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the hazard of being struck by manual tongs that
were deteriorated
and not kept in safe operating condition.
(a)Cyclone Drilling & Drill #5, Piceance Creek, Meeker, CO: On or before
January 02, 2003, the employer did not perform on-the-job training and
adequately instruct each employee in safe working practices and the
recognition and avoidance of unsafe condition associated with gas well
drilling
operations. An untrained employee, who was involved in gas well drilling
operations, sustained a severe blow to the head when the upper tong latch
slipped out of the tong head jaws.
Among other methods, one feasible and acceptable abatement method to
correct the hazard
is to implement an effective safety program.  Such safety training
programs should include,
but is not limited to the following: Instruction of the driller's or crew
chief's responsibility
for the safety of the crew and equipment during normal operations and
possible emergencies.
Indoctrination of new personnel. New rig personnel, regardless of prior
experience, should
have the job outlined, explained, and demonstrated by the driller, crew
chief, or another
designated crew member. Observation of new personnel work performance
should be
maintained until the applicable supervisor is satisfied that the personnel
can fill the position
in a safe and effective manner. Instruction of crew members on work
procedures and safe
practices should be given.  This is required by American Petroleum
Institute, API
Recommended Practice 54 1999, Section 6, Sub-Section 6.1, Paragraph 6.1.7
a-g, page 9.
