Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falls from elevated work stations while using single point suspension scaffolding, due to improper rigging on or about 10/30/02. The tail end of the wire rope did not go to the ground or, in the alterative, was not secured to the stirrup. Among others, one feasible and acceptable method to abate this hazard is to follow the manufacturer's rigging guidelines.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falls from elevated work stations while using single point suspension scaffolding, due to the use of wire rope that was not acceptable for such use on or about 10/30/02. The wire rope did not meet Power Climber manufacturer's specifications, in that it was 4x26 class instead of 6x19 or 6x31 class. Among others, one feasible and acceptable method to abate this hazard is to follow the manufacturer's rigging guidelines.
