Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crushing and fall hazards at the three (3) material lifts
(rams), as follows:
a.Establishment:  At all three (3) material lifts (two down rams and
one up ram) employees were able to access the upper shaft areas,
exposing themselves to falls of approximately 20 feet. The
employees were exposed to fall hazards when opening the ram
doors, loading rolls of material, or checking on that status of the
rolls of fabric in the shaft below.
Employees were also able to access the lower shaft areas of all
three lifts (rams), exposing themselves to the hazards of being
struck by and/or caught between the moving platform or forks.
Employees were also exposed to the hazard of being struck by
moving rolls of materials.
Feasible and useful methods to correct these hazards, among other
methods, are the installation of gates or other devices with
interlocking mechanisms, that would prevent the employees from
accessing the shaft from above and below.  Also provide training
and instruction to employees using these rams on the dangers of
entering the shafts. In addition, it is recommended that the
employer assure compliance with state and local ordinances, as
well as ANSI/ASME A17.1, The American National Safety Code
for Elevators, Dumbwaiters, and Moving Walks.
