fatality case wherein the victim was crushed as a result of the rear

vehicle backing up unexpectedly as the driver was not aware that the truck
was in reverse gear.
In that the back-up alarm was inoperative.
Feasible abatement would be to test all safety equipment prior to each use
on the rear loading
garbage collection vehicle.  This includes testing the back-up alarm.
