employer failed to
provide a place of employment which was free from recognized hazards that
were likely to
cause death or serious physical harm to employees:
a. On or about February 14, 2002, at the jobsite located at Clybourn
Avenue and Diversey
Avenue in Chicago, Illinois, employees working from a platform supported
by a rough
terrain forklift truck were exposed to injuries from falls due to improper
elevating of
personnel. The proper method is required by the American National
Standards Institute's
standard for rough terrain forklift trucks. (ANSI B56.6-1992).
Abatement documentation is required for this item in accordance with the
requirements of 29
CFR 1903.19(d).
