causing or likely to
cause death or serious physical harm to employees in that employees were
exposed to:
a)falls from elevation. Employees of Sofia Enterprises utilized a forklift
on the
South Wing of the Chatham Woods project, Elkin, NC, equipped with a trash
box, to reach heights approximately 42 feet in the air, prior to and on
May 29,
2002. The forklift is not designed by the manufacturer to be used for
lifting
employees.
