place of employment free from recognized hazards that were causing or
likely to cause death or
serious physical harm to employees in that employees were exposed to:
a)the hazard of being struck by and pinned beneath the Toyota, Model 786A
industrial truck due to the absence of an active operator restraint system
or seat
belt.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard would be the installation of the seat belt retrofit kit made
available by the powered
industrial truck manufacturer or distributer and requiring the operator to
wear same.
