Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the possibility of an exploding grinding wheel from grinding on the side of the wheel. a) Army's Transmissions, 1080 Forest Avenue, West Homestead, PA. Employees were exposed to severe trauma up to and including death from grinding on the side of the grinding wheel of the Black and Decker Industrial model 1788, serial # 4060321 3600 rpm pedestal grinder located in the rest room/storage room in the shop. 03-07-02. Abatement note: A feasible and useful means of abatement, among others, is to train employees not to grind on the side of the wheel and to post a sign stating " DANGER, do not grind on the side of the wheel" above and to the rear of the grinder.
