Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish to
each of his employees employment and a place of employment which were free
from recognized
hazards that were causing or likely to cause death or serious physical
harm to employees:
Andras Corporation dba Cromco Burial Vault, Elyria, OH:  Employees were
exposed to the hazard of
being struck-by the load while handling burial vaults:
1.)  The 2-ton Louden underhung crane's pendant control box was damaged
exposing the push-
buttons that maneuver the crane, as observed on or about May 23, 2002.
Among other methods, one
feasible and acceptable abatement method to correct this hazard is to:
Conduct frequent and periodic
inspections of the crane system, document critical items identified during
the inspections, and correct
any condition disclosed by the inspections that is determined to be a
hazard to continued operation by
repair or replacement before continuing the use of the equipment in
accordance with ASME B30.11-
1993, "Monorails and Underhung Cranes."
2.)  The spreader bar had no load capacity rating and was not being
inspected on a frequent and
periodic basis, as observed on or about May 23, 2002.  Among other
methods, one feasible method
of abatement is to: Conduct frequent and periodic inspections of the
lifting device and mark the lifting
device with the rated load capacity in accordance with ASME B30.20a-2001,
Below-the-Hook
Lifting Devices.
