Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees were exposed to: Hazard of explosion and/or fires in the Spring Room due to the absence of a control means on the forge furnace to assure that flame supervision is maintained in the event of flame failure. Among other methods, one feasible and acceptable method to correct this hazard would be the installation of burner shutdown and flame supervision controls, following the precepts of NFPA 86, Standards for Ovens and Furnaces, 1999, section 5-9.1, which states: Each burner flame shall be supervised by a combustion safeguard having a maximum flame failure response time of 4 seconds or less, that performs a safe-start check and is interlocked into the combustion safety circuitry. Additional abatement information is required for this item.
