Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees.
a)On or about March 27, 2003 at Oracle Packaging, Inc., Toledo, OH,
employee(s) were
exposed to the hazard of being struck by the paper rolls weighing
approximately  2 3/4 tons as
the rolls are lifted onto the slitter to be cut.
The three ton Slitter Hoist had not been inspected on a frequent and
periodic basis. Among other
methods.
A feasible and acceptable abatement method to correct hazard(s) is to:
Conduct frequent and
periodic inspections of the crane system, document critical items
identified during the
inspections, and correct any condition disclosed by the inspection(s) that
is determined to be a
hazard to continued operation by repair or replacement before continuing
the use of the
equipment in accordance with ASME B30.11-1993, "Monorails and Underhung
Cranes".
b)The spreader bar had not been inspected on a frequent and periodic
basis.
The rated load capacity, after making repair(s),was not determined then
marked on the spreader
bar.
A feasible and acceptable method of abatement would be to: Conduct
frequent and periodic
inspections of the lifting device and mark the lifting device with the
rated load capacity in
accordance with ASME B30.20a-2001," Below-the-Hook Lifting Devices".
