employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to injury from not using seat belts while operating powered
industrial trucks:
(a) Worldwide Refractories, Inc.:  On or about 7/23/03, employees were not
wearing
seatbelts while operating powered industrial trucks.
AMONG OTHER MEANS, ONE FEASIBLE MEANS OF ABATEMENT IS TO ENSURE
THAT EMPLOYEES ARE WEARING SEATBELTS WHEN OPERATING POWERED
INDUSTRIAL TRUCKS.
