Section 5 (a) (1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment or a place of employment which was free from recognized
hazards that are
causing or likely to cause death or serious physical harm to employees, in
that employees were
exposed to the hazard of being struck by or caught in hydraulic presses:
In the Die Cast Department, Trim Presses,
a.  Employees who operated hydraulic presses did not receive adequate
training in the
proper care and use of the press,
b.  Employees who performed maintenance on hydraulic presses were not
properly
trained or observed to ensure their competence,
c.  Two hand controls were not protected against unintended operation and
did not
require the concurrent use of both hands to initiate the press stroke,
d.  Safety blocks were not used to prevent slide motion while performing
maintenance
or set up,
e.  The employer did not establish and follow a systematic program of
periodic and
regular inspection of their hydraulic presses, and
f.  The control system did not prevent an unintended stroke of a hydraulic
press.
Among other methods, one feasible and acceptable abatement method to
correct these
hazards is to comply with ANSI B11.2-1995, Standard for Machine Tools -
Hydraulic
Power Presses - Safety Requirements for Construction, Care and Use:
1. By establishing and following a program of periodic and regular
inspections and
maintenance for hydraulic presses,
2. Training all operators, setup personnel and maintenance staff in the
proper care and
use of Hydraulic Presses,
3. Inspect and require that all two hand controls be protected against
unintended
operation,
4. Require the concurrent use of both hands to initiate the press stroke,
5. Require the use of safety blocks when personnel put any part of their
body between
the ram and bolster plate or between the dies.
6.  Ensure the control system is designed and constructed so a single
failure or fault
within the system does not prevent the normal stopping action from being
applied to the
press, does not create unintended slide motion and does prevent initiation
of a successive
stroke until the failure is corrected.
