employees were exposed to being struck by falling loads due to the failure
to properly rig the
hoisting slings.
In the boiler room at Glenbard West High School, 670 Crescent Boulevard,
Glen Ellyn,
IL, the chain fall being used to hoist the expansion tank into place was
not properly
rigged as specified in ASME/ANSI B30.16-1987 Section 16-3.1.2a in that the
secondary
hoist chain was wrapped around the load.
A feasible method of abatement would be to properly rig the chain hoists
as specified in
ASME/ANSI B30.16-1987 Section 16-3.1.2a.
