recognized hazards, that
were likely to cause death or serious physical harm to employees, in that
employees were
exposed to being struck by unsecured loads from company-fabricated lifting
devices which have
failed as a result of over-loading:
On or about October 3, 2002, employees in the Hinge Room at S & D
Products, Inc.,
were using company-fabricated lifting devices to lift metal coils.  The
lifts were not load-
rated nor were they tested to ensure that they were capable of safely
lifting the load.
A feasible form of abatement would be to comply with ANSI Standard, ASME
B30.20b-
1996, Below-The-Hook Lifting Devices.
