Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment an a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation from the lack of specific work rules and training designed to prevent the of an employee to the hazards of threading nylon cord through the winder of the extruder machinery: Production Area: On or about 2/02/03, an operator was tying a knot in a piece of nylon cord that was threaded through the winder of extruder #5 and used to pull the plastic film through the winder. The nylon cord wrapped around the idler rollers and cinched around the employees fingers.There were no specific work rules and/or training designed to prevent the exposure of the employees to the hazard of the operation and /or the hazards that the rope, extruder and idler roller could cause as it related to the knot tying procedure. Training of the employee did not address the possible hazard of the cinched cord becoming wrapped around the employees hand. Safety trip controls were not located to stop the process in case the operator's hands became wrapped in the cord. Among feasible abatement methods several methods would be to: 1. Change the training manual to address threading the winder without cinching material to pull the plastic film through the winder. 2. Ensure trainees are shown the proper procedures for threading the winder of the extruder. 3. Change the material used for threading to a break-away or easily torn material. 4. Ensure that all current and future operators are shown the proper procedures for threading the extruders. Ensure that safety trip controls are within reach of the operators during all phases of the work that they are performing. CERTIFICATION AND DOCUMENTATION REQUIRED FOR THIS ITEM.
