Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to the hazard of burns from igniting the kerosene soaked rags during the process of lighting the burners of the annealing furnaces, and purging the liners: a) On or about November 14, 2002, and at times prior, employees were exposed to serious burns when using a cigarette lighter to ignite kerosene soaked rags attached to poles. Kerosene flowing down the poles could contact employees and ignite, exposing employees to burn hazards. Among other methods, one feasible and acceptable abatement method to correct these hazards is to repair the self striker igniter on the burners of the annealing furnaces and use an acetylene torch to purge the liners. Abatement Verification: Abatement certification and documentation is required for this item (see enclosed "Sample Abatement-Certification Letter").-Certification

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to a crushing hazard: a) On or about November 14, 2002, and at times prior, in the annealing furnace area the lifting device used to lift the liners was not marked with a rated load marking. Among other methods, one feasible and acceptable abatement method to correct this hazard is to ensure that the lifting apparatus is tagged with an appropriate rated load marking in accordance with ANSI/ASME B30.20a-1987. Abatement Verification: Abatement certification and documentation is required for this item (see enclosed "Sample Abatement-Certification Letter").

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, the use of seat belts by employees operating forklifts was not enforced, exposing employees to crushing injuries in the event of a forklift tip-over or other accident: a) On or about November 14, 2002, and at times prior, employees operated forklifts while not wearing seat belts exposing them to serious injuries in the event of a tip-over or other accident. Among other methods, one feasible and acceptable abatement method to correct this hazard is to ensure that employees are trained regarding the hazards associated with not wearing seat belts and to implement an active, effective enforcement policy.
