Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees working in the Exhauster building near the 300 HP steam turbine and in the By-Products pump house near the 60 hp steam turbine were exposed to the hazards of being struck by flying parts and/or the release of steam due to overpressurizing the Worthington 60 Hp and the Terry 300 Hp single stage steam turbines leading to explosion of the casings and/or components in that: (a) On the Terry steam turbine located in the Exhauster Building, there was no speed governor attached to ensure that the turbine was not being operated under overspeed conditions. (b)Daily, weekly and monthly inspections and annual maintenance of turbine components and safety devices including overspeed devices were not conducted on the single stage steam turbines such as the Terry GA 300 hp and the Worthington R2R 30 hp. Amongst feasible abatement methods, follow Dresser-Rand manufacturer general operation and maintenance manuals for single stage steam turbines.
