Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer failed to provide a place of employment which was free from recognized hazards that were likely to cause death or serious physical harm to employees: a. On or about April 23, 2002, at the jobsite located at 4300 North Marine Drive in Chicago, Illinois, employees working on a two-point suspended scaffold were exposed to injuries from falls due to improper use of outrigger beams. The proper usage of scaffold outriggers requires a 950 to 800 lb counterweight to be used on each outrigger beam with a 45 to 38 inch overhang. This information is provided by the manufacturer in the manufacturer instructions. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
