Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Press Brake Area - The Di-Arco Hydra Power Press Brake #14-72 had foot pedals used by employees while bending metal. The foot pedals were not adequately covered to prevent unintended activation. Employees were exposed to amputation hazards from the point of operation. Among other methods one feasible and acceptable method to correct this hazard is to include, but not be limited to the following: (1) Conform the American National Standard Institute B11.3-2002, section 6.11.3.1 Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
