Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being entangled in machinery: a) Employees were provided with a Rigid 300 portable power driven pipe threading machine that was not equipped with a momentary contact device/constant pressure treadle control switch. A feasible means of abatement would be to provide a properly guarded momentary contact/foot treadle control switch as required by American National Standards Institute (ANSI) B208.1 - 1982, Section 4.4. Abatement certification required for this item in accordance with 29 CFR 1903.19(c).1n

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falling: a) Employees were exposed to the hazard of falling approximately 22 feet while working from a JLG 35 electric extendable boom lift without the benefit of fall protection required by the American National Standards Institute (ANSI) A92.5 - 1992 Standard for Boom - Supported Elevating Work Platforms. A feasible means of abatement would be to ensure the use of fall protection as required by American National Standards Institute (ANSI) A92.5 - 1992. Abatement certification required for this item in accordance with 29 CFR 1903.19(c).
