Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: crushing injuries caused by cave-in of the earth embankment. The embankment was vertical and approximately 30 feet high and the soil was wet. a) Tony Columbo Route 6 West, Tunkhannock, Pa: Employees were exposed to cave-in hazards while installing concrete footers when working near an unprotected 30 foot high dirt embankment. On or about March 15, 2004 ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIREDATION
