Among others, one feasible and acceptable method of abating the above conditions would be to: a)Establish and implement a system for the investigation of any and all abrasive wheel failures to thoroughly assess the failure and discover the root cause of any such failures for the purpose of correcting the equipment, procedure, or practice that resulted in the failure; b)Establish and implement a preventative maintenance program in accordance with the manufacturer's recommendations and American National Standards Institute (ANSI) B11.9; c)Establish and implement a storage location and method for storing abrasive wheels in accordance with the wheel manufacturers recommendations and ANSI B7.1; d)Repair/replace the safety switch on the Fox Swing Frame grinder and operate the grinder in accordance with the grinder manufacturer's standard operating procedures.
