were exposed to the hazard of pieces of metal being broken off of the
hydraulic press ram and
being propelled away from the press:
a)South Work 800 ton hydraulic press, Hannifin 50 ton hydraulic press
number 0905, and hydraulic press 0801:  The ram heads on these presses
had mushroomed and were not dressed to prevent pieces of the
mushroomed area from being broken off and propelled away from the
press, presenting the hazard to employees of being struck by the flying
projectiles.
Among others, feasible abatement for this hazard is:
1.Monitor the ram heads for mushrooming or other damage
and the make the necessary repairs.
2.Establish ram head inspection criteria and train operators
on the proper inspection procedures.  The ram heads
should be inspected prior to beginning operations each day.
Certification, and documentation/evidence of abatement is required.
Certification of abatement is required by 29 CFR Part 1903 and is to be
submitted within ten (10)

of the press.
2.Provide guarding of the foot pedal to protect it from
unintended actuation from falling objects, inadvertent
actuation by the operator, or any other inadvertent
actuation.

On or about August 6, 2002, in the Cleaning Room, employees were using the
Fox Swing
Frame grinder, serial number 5986364, and were exposed to the hazards of
abrasive wheel
failure as a result of:
a)The employer did not properly, thoroughly, and timely investigate the
cause of a past
abrasive wheel fracture on the Fox Live Stage Swing Frame grinder which
occurred on
or about August 2, 2002;
b)There was no preventative maintenance program to detect wear outside of
tolerance
limits and defects such as cracked guards as existed on the Fox Swing
Frame grinder
in the Cleaning Room;
c)Abrasive wheels used on the Fox Swing Frame grinder and other grinders
were not
stored in a suitable location in racks, bins, drawers or boxes, and were
subject to high
humidity or water, the potential of being struck or bumped by moving
vehicles
such as fork lifts, and being knocked over accidentally;
d)A safety switch on the Fox Swing Frame grinder was not operational which
would
have prevented the spindle of the grinder from operating at speeds greater
than the safe
operating speed for the abrasive wheel mounted on the grinder;
e)The abrasive wheel on the Fox Swing Frame grinder was operated at 2420
RPM when
the manufacturer's recommended safe operating speed for that wheel
diameter was
1890 RPM.
Among others, one feasible and acceptable method of abating the above
conditions would be
to:
a)Establish and implement a system for the investigation of any and all
abrasive wheel
failures to thoroughly assess the failure and discover the root cause of
any such
failures for the purpose of correcting the equipment, procedure, or
practice that
resulted in the failure;
b)Establish and implement a preventative maintenance program in accordance
with the
manufacturer's recommendations and American National Standards Institute
(ANSI)
B11.9;
c)Establish and implement a storage location and method for storing
abrasive wheels in
accordance with the wheel manufacturers recommendations and ANSI B7.1;
d)Repair/replace the safety switch on the Fox Swing Frame grinder and
operate the
grinder in accordance with the grinder manufacturer's standard operating
procedures.
Certification, and documentation/evidence of abatement is required.
