Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that:
(a)On or about September 19, 2002, at the jobsite, located in the
parking garage at 209 E. Mason Street, Milwaukee, WI, an
employee (electrician), engaged in the installation of electrical
lighting upgrades, on the sloped (10 to 15 degrees) driveway ramp
to the basement parking lot and while elevated 3 feet and
traversing the sloped driveway in reverse, in a Skyjack SJIII
Model 3015, Compact, Battery-Operated, Self-Propelled Mobile
Elevating Work Platform, (Scissor Lift), the scissor lift tipped over
injuring both of the employee's arms.
Among others, one acceptable method of abatement would be to
follow procedures specified in the ANSI/SIA A92.6-1999, for Self-
Propelled Elevating Work Platforms, specifically those identified
in sections 4.6.6 Slope Warning, 8.6 Before Operation items (1)
through (3), also section 8.7 Workplace Inspection items (1)
through (9) and section 8.10.
No certification required.  Violation Abated.
