furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that the
employee(s) using the TB-44 Boom-Supported Elevating Work Platform were
exposed to a
fall/thrown from hazard of 15 feet:
(a)On or about November 15, 2002, the employer did not ensure that
the employee(s) accessing the roof by way of the boom-lift, for the
purpose of removing plywood at the job site located at the Kettle
Moraine High School, addition and renovation project in Wales,
WI, were not provided with protection from falls.
A feasible means of abatement, among others, would be to follow
the ANSI/SIA A92.5-1992, section 8.8(g) "Ensure all personnel in
the platform are wearing fall protection devices and other safety
gear at all times."
No certification required.  Violation Abated.on
