Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: Employees were exposed to loads becoming detached from a hoist during the lifting of loads. On or about November 25, 2002: A.While lifting and flipping dies using hoists, employees were exposed to the hazards of loads braking free and falling on the employees. B.Hoists were not inspected in accordance with the manufacturer's recommendations. C.A preventive maintenance program was not established in accordance with the manufacturer's recommendations. Among other methods, feasible and acceptable methods of abatement to correct these hazards, include, but are not limited to, the following: A.Establish and implement a training program for the operation of hoists, including proper methods to be used in flipping dies, in accordance with ANSI B30.16-2.3 - 1973 and the manufacturer's recommendations. B.Establish and implement an inspection program, including completion of and retention of inspection records, in accordance with ANSI B30.16-2.2.1 - 1973 and B30.16-2.2.1.5 - 1973 and the manufacturer's recommendations. C.Establish and implement a preventive maintenance program in accordance with ANSI B30.16-2.2.3 - 1973 and the manufacturer's recommendations. Certification, and documentation/evidence of abatement is required. Certification of abatement is required by 29 CFR Part 1903 and is to be submitted within ten (10) calendar days after the abatement date.
