Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employee(s) were
exposed to caught-in hazards from rotating parts:
(a)On or about August 1, 2003 employee(s) were operating a GOMACO GT-6000
slipform paver/trimmer without the trimmer head guarding in place. This
exposed
employee(s) to a caught-in hazard in that the trimmer wheel front steel
plate rock
guard and attached rubber flap was not attached to the front of the
trimmer head.
One feasible means of abatement would be to ensure that the trimmer head
front rock guard is
secured and in place, and establishing a procedure that requires the
automated trimmer head to
be stopped and disengaged when mounting and dismounting the slipform
paver.
Certification, and documentation/evidence of abatement is required.
Certification of abatement is required by 29 CFR Part 1903 and is to be
submitted within ten (10)
calendar days after the abatement date.
