Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to death or the potential for serious crushing and broken bone injuries from falls: (a)On or about August 6, 2003, employees were exposed to fall hazards while working from a pallet suspended approximately 18' in the air by a Crown Order Picker Model SP3000, S/N 1A247499, and no provision had been made to insure that the pallet was secured to the forks of the equipment. Among other methods, one feasible and acceptable method of correcting this condition is to follow the practices specified in ANSI B56.1 - 1993, paragraph 4.17 which requires, among others, that platform attachment means (of a lift truck) are applied and the platform is securely attached to the lifting carriage or forks. Certification, and documentation/evidence of abatement is required. Certification of abatement is required by 29 CFR Part 1903 and is to be submitted within ten (10) calendar days after the abatement date.ment
