12-60-2(a)(3)Each employee was not provided a safe work place and practices by elimination or reduction of existing or potential hazards, i.e., full body harnesses and lanyards were not provided for the six employees that operated a Clark Order Picker. Body Belts with visible signs of wear and tear with 5 foot ropes were provided to employees who stepped onto the wooden skip to retrieve items. Abatement documentation such as written, videographic, or photographic evidence of abatement is required. Location: Establishment Warehousement
