Section 5(a)(1) of the Occupational Safety and Health Act of 1970: the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to crushing and amputation hazards caused by the unintentional operation of an unguarded foot pedal of; A) Chicago Mechanical Press Brake, No. NR052, located in the brake and shear department; B) Chicago Dreise & Krug press brake, No. NR053, located in the brake and shear department. An unguarded foot pedal on a press brake could be activated by falling objects or accidental operator contact. One feasible and acceptable abatement method to correct this hazard is to guard the foot pedal as referenced in ANSI B11.3-1982, Safety Requirements for Construction, Care, and Use of Power Press Brakes, by providing a locking pin or locking lever or installation of a protective cover over the foot-pedal.

General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to crushing hazards caused by stair landing sections falling from the monorail transport system. The stair landing sections were not properly suspended with both legs of the pressure gripping hooks engaging the load. Only one leg of the pressure gripping hooks supported the load, and should the stair landing section be bumped by an employee or powered industrial equipment it would be knocked from the suspension hooks. When properly engaged the weight of the suspended load causes both of the suspension hook legs to be drawn in tight against the load, distributing the load between both legs. One feasible and acceptable abatement method to correct this hazard is to use proper suspension/load bearing devices designed to properly engage and support the stair landing sections.
